From: ENMU Monday Memo Subject: FW: Important Fringe Benefit Information The New Mexico Public Schools Insurance Authority is offering the following switch enrollment opportunities. Premium changes for these adjustments will be made on the December payroll deductions for coverage changes effective January 1, 2010. Health Insurance Switch Enrollment Employees who are currently participating in one of the ENMU health insurance plans now have the opportunity to "switch" their enrollment between plans. The health plans offered are Blue Cross/Blue Shield and Presbyterian. Employees may also "switch" their coverage options between high and low during this time as well. Employees interested in "switching" health insurance plans or coverage options should complete a change form in the Office of Human Resources, Admin. 113, by Friday, November 20, 2009. Dental Insurance Open Enrollment Open enrollment for United Concordia dental insurance is now being offered to employees who are eligible for insurance benefits but are not currently enrolled for dental coverage. Employees wishing to enroll for dental insurance during open enrollment may choose between the United Concordia high and low options. Applications should be completed in the Office of Human Resources, Admin. 113, by Friday, November 20, 2009. Proof of eligibility is required for all dependents. Documents required are marriage license for spouse coverage and birth certificates for dependent children. Dental Insurance Switch Enrollment Employees who are participating in an ENMU United Concordia dental insurance plan now have the opportunity to "switch" their coverage option between the high and low options. Employees interested in "switching" dental coverage options should complete a change form in the Office of Human Resources, Admin. 113, by Friday, November 20, 2009. Vision Insurance Open Enrollment Employees who are not currently covered by the ENMU Vision Insurance Plan (Davis Vision) may now enroll for 2010 coverage. Applications for December deductions (January 2010 coverage) must be completed in the Office of Human Resources by Friday, November 20, 2009. Medical Premium Tax-Shelter Plan All employees who currently have medical, dental, or vision insurance coverage have the option to change their tax-shelter status. Tax-shelter forms are available in the Office of Human Resources and are also available on the Intranet under Human Resource Forms. UNUM Life Insurance Re-Enrollment All employees who are currently participating with UNUM Life Insurance may increase or decrease their coverage. If you want to increase your coverage, you will need to complete an evidence of insurability form for medical underwriting review. Employees interested in increasing or decreasing their UNUM Life Insurance coverage should complete a change form in the Office of Human Resources by Friday, November 20, 2009.
