2003 NOMINATION FORM
Portales/Ruidoso
Deadline for Nominations: 5:00 p.m., Friday, March 28, 2003
Nominations are accepted in three categories: Faculty, Professional Staff, and Support Staff. One person may nominate as many people as desired in any of the categories. Please use a separate form for each person nominated. (All administrators and individuals who have received the award within the past 3 years are not eligible to receive a President's Spirit of Eastern Award.) Additional forms may be obtained at the Portales Campus from the University Relations Information Desk, the President's Office or at the Ruidoso Instruction Center office.
Please complete the following information. Forms with incomplete responses will not be considered.
Return completed forms to: Awards Committee, President's Office, Station #1.
