This form is to be completed by faculty teaching freshman-level courses. Please complete one form for each freshman student whose academic performance is unsatisfactory. Use this online form, or download the available PDF to type in and print. If you choose to use the PDF, please return the completed form to the Office of the Freshman Year, Station 29. Your envelope should be marked “confidential.”

This information is not to be given to anyone except the student and authorized University officials. Please print a file for your records using the "print" button following the form.
