Job Summary: Due to our expanding business, UnitedHealthcare has an outstanding opportunity available for a Service Coordinator in the Santa Fe, NM area (including Espanola and Las Vegas).

This is a field based/telecommuting role that requires 80% local travel in theSanta Fe/Espanola/Las Vegasarea.

Qualifications: Bachelor's degree in Social Work or other related field OR Associate's Degree in Nursing
Minimum of 1 year clinical or case management experience
Minimum of 1 year experience in long-term care, home health, hospice, public health, or assisted living
Basic level of proficiency in PC-based word processing and database documentation (Word, Excel, Internet, Outlook)
Valid Driver's License, access to reliable transportation and proof of automobile insurance

Behavioral Health or RN license in New Mexico
Medicare/Medicaid experience
A background working in geriatric special needs
A background in behavioral health
Experience with electronic charting
Bilingual in English/Spanish is a plus

Duties: Engaging members telephonically and in-personto coordinate services, community resources, and treatment needs

Delivering a holistic approach to coordinated care based on the enrollee needs

Identifying early risk factors and conducting ongoing assessments and documenting in an electronic file

Creating, reviewing and revising Plans of Care and focusing on disease management

Functioning as an advocate for members and decreasing unnecessary hospitalizations

Collaborating with the member's PCP to deliver and coordinate necessary services

Building relationships with members and their families; assisting them with proper health choices

Ensuring cases are documented in a timely manner
