Name, Social Security and Address Change form is no longer available online. Name, Social Security changes must be done through the Office of the Registrar.
Address changes should be made in Self Service Banner each semester (Banner login)
- FERPA
- Drop/Add Form
- Late Drop/Add Withdrawal Form - allows student to drop/add courses after the deadline and to withdrawal due to extreme circumstances
- Enrollment Approval Form
- Name or Address Change (through Banner)
- Petition for Readmission - allows the student to petition return to school after a suspension within the sitting out time
- Registration Form
- Special Study Courses Approval
- Student Petition for Waiver of Academic Policy - allows students to petition a waiver of a specific academic policy such as degree requirements
